Custom Forms Overview

Valant Custom Forms enables practices to:

  • build advanced forms
  • deliver the appropriate documents to patients based on the services they seek
  • have patients review and accept practice policies
  • collect consent signatures
  • track completion status across the practice
  • acquire detailed patient information at intake

Practices with Premium or Power subscriptions have access to Valant Custom Forms. This feature includes no fees to send forms or packets for signatures.

Custom Forms replaces the Dropbox Sign eSignature solution, and existing practices that use eSignature will need to rebuild eSignature Eligible forms in the Custom Forms Hub before Dropbox Sign is disabled. 

Steps to move eSignature documents to Custom Forms:

  • Determine which documents need to be rebuilt
  • Use Word, pdf, or other desired format version to copy and paste into Custom Forms
  • Use the Duplicate Form "Action" to decrease rebuilding time
