Automated packet assignment reduces administrative burden, improves efficiency, and ensures paperwork is sent and filled out in a timely manner. Custom Forms users are able to automatically send forms to either "All Patients" or patients, guarantors, guardians, and/or related persons based on which Program(s) the patient is enrolled in.
Permission and Subscription Level needed for access:
- The practice must have the Power or Premium subscription
- The user must have the "Custom Forms: Forms, Packets, Automation" permission

Adding an Automated Request
Navigate to Documents | Custom Forms Hub, click Automation
Click + Add from the Automated Requests page
Populate the required fields and any additional appropriate fields
- Select a Program: required, choose from the drop-down options
- Select a Packet: required, choose from the drop-down options
- Primary Recipient: required, usually the patient, select from the drop-down options
- Additional Signers (If available): not required, select from the drop-down options
- Reminder Cadence - How often an email is sent out: required, select from the drop-down options
- Due Date - How many days do they have to complete the form(s): required, select from the drop-down options
- Patients created after: not required, select an appropriate date
Click Save
Managing an Automated Request
Navigate to Documents | Custom Forms Hub, click Automation
Select from the list and choose from the Action drop-down options
- Edit: opens the request window, make any desired edits, click Save
- Delete: removes the request
- History: shows any edits or updates made to the request
FAQ
What triggers sending a package?
Since forms can be completed in MYIO or via email, the patient needs an active MYIO account or an email address listed in the Patient Communications field.
What are use cases for which a practice might use automated packet assignment?
- Sending automated intake packets to patients as they are onboarded
- Automated forms sent to patients newly enrolled in a program
- Use the "Patients created after" date to send a packet to any new patients who began after a specified date
Does a patient need to be in a program to receive an automated packet assignment?
No, the practice can use "All Patients" from the "Select a Program" field for patients whether or not they are in a program.
