Adding a Fax Number to a Lab Request

When creating a paper lab request, the information at the top of the form populates by default from the practice information (practice name, provider name, practice address, practice phone number). If a fax number is desired to be on the form, the following steps should be completed.

  1. Click on Persons & Institutions | Providers
  2. Select a provider
  3. Click the Preferences tab
  4. Uncheck the "Use Practice Address for Lab Requests" checkbox
  5. Click on the Provider tab
  6. Verify or enter a fax number for the provider

The form will now populate the information from the provider (practice name, provider name, provider address, provider phone number, provider fax number).
