When creating a paper lab request, the information at the top of the form populates by default from the practice information (practice name, provider name, practice address, practice phone number). If a fax number is desired to be on the form, the following steps should be completed.
- Click on Persons & Institutions | Providers
- Select a provider
- Click the Preferences tab
- Uncheck the "Use Practice Address for Lab Requests" checkbox
- Click on the Provider tab
- Verify or enter a fax number for the provider
The form will now populate the information from the provider (practice name, provider name, provider address, provider phone number, provider fax number).
