AHS – Adult Hope Scale

Description  

The Adult Hope Scale is a 12-item questionnaire designed to assess the respondent’s level of hope.

Special Instructions  

Once the patient has submitted it, you may preview it on the uninitialed documents tab in the EHR, and initial it to save it to the Patient Chart. If you want it to be pulled into your next Mobile Note, then do not initial it.  

As recommended by the authors, the heading when this scale is viewed in MYIO and Mobile Notes is “The Future Scale”.

Scores Produced  

  • Pathways Subscale Score (4-32). Higher scores indicate higher levels of pathways thinking.
  • Agency Subscale Score (4-32). Higher scores indicate higher levels of agency thinking.
  • Total Score (8-64). Higher scores indicate higher levels of hope.
